GM does do that! Look at the Gen III/Gen IV V8 engine line: All shapes sizes and colors.
As for increasing valve size, changing a cam, etc., this stuff isn't cheap or easy when your dealign with mass production. There's tooling (or at least purchasing) costs for even the most simple engine changes. On top of that, all changes need to be validated through thorough testing and certified by the EPA.
It's easy for your shade-tree mechanic to do this, as he only has one car to modify. He also doesn't have Unlce Fed regulating what he does to his car, assuming that he's emissions compliant.
